The Next Era of Healthcare Is Personal: Better Outcomes with Personalized Care Management Programs

The industry-wide focus on value-based care continues to gain momentum. Payers and leading healthcare providers are shifting their attention to broader populations with an eye toward maintaining better health for more people now, rather than  treating a disease after it has already  impacted a patient’s well-being or quality of life.

This shift comes at just the right time. Chronic disease impacts over half of Americans, and associated costs are projected to reach $47 trillion globally by 2030.

The rising impact of these conditions is partly due to the fact that, historically, healthcare payers and providers have had to wait for a condition to present itself before putting a patient on a treatment plan. However, the rise of smart tools and powerful analytics is changing this.

Predictive analytics, combined with Chordline’s comprehensive Clinical Care Management Programs, help payers and providers identify risk factors for patients. By equipping the care team with the right insights and enrolling patients in automated, personalized care plans, providers can intervene earlier, strengthening patient outcomes and lowering costs.

A More Personalized Look at Patient Risk

A key reason this approach isn’t currently widely used is that providers and payers don’t have time to individually screen patients, then study the patient’s current health, history, social determinants of health and other factors that could point to a risk of disease. Also, many current screening methods, such as common risk assessments, don’t take a patient’s social or cultural needs into account, which can exclude some patients from participating.

Chordline’s flagship Health Risk Assessment (HRA) is designed to intentionally and inclusively evaluate a patient’s health behaviors and risk factors to capture this information in a way that is accessible to patients and insightful for providers.

Having members meet face-to-face with care teams for an assessment can be cumbersome, time-consuming and resource intensive. Chordline’s HRA can easily be sent to a member via a secure link, allowing them to complete the assessment at their convenience. Not only does this provide a better overall experience, but it also allows the patient to take their time to consider and correctly recall relevant details of their health history. Ultimately, this creates a more accurate representation of their health.

 Moving from Insight to Action—Automatically

While the HRA is a critical first step in a patient’s care program, it’s only the beginning. Chordline’s HRA process auto-generates a customizable care plan unique to each patient. The care plans include an outline of patient-specific problems, goals, interventions and outcomes. Clinicians can use these plans as a roadmap for decision-making and can easily update and refine the plan to align with the member’s changing needs and circumstances.

The automated creation of these highly personalized care plans eases an enormous burden from the care team, but Chordline takes this a step further.

We have created six specialized care management programs to tackle common health challenges and enhance member well-being. Depending on their HRA and care plans, members can be automatically enrolled in these programs to help them more proactively and powerfully manage their conditions.

The programs, built in accordance with NCQA and URAC standards, include:

·        Asthma Care Management Program: Our Asthma Care Management Program helps members identify asthma triggers while offering tailored respiratory health strategies to reduce symptoms.

·        COPD Care Management Program: This program provides comprehensive support for COPD management, enhancing breathing health and quality of life.

·        Coronary Artery Disease (CAD) Care Management Program: Our program guides members through CAD management, giving them customized interventions focused on heart health and risk reduction.

·        Diabetes Care Management Program: The program empowers members to manage diabetes with personalized plans that support glycemic control and lifestyle adjustments.

·        Heart Failure Care Management Program: The heart failure program offers a dynamic approach to care management, supporting symptom relief and cardiovascular wellness.

·        Pregnancy Care Management Program: Following members from pregnancy through post-partum, this program includes high-risk identifiers and continuous monitoring to ensure safe, healthy care journeys.

As healthcare stakeholders continue to move further into value-based care, it is critical that payers and providers both find ways to automate administrative tasks while providing patients with a more personalized experience. A comprehensive, inclusive HRA paired with impactful care management programs can help stakeholders more quickly identify and treat common conditions, ultimately lowering costs while boosting efficiency and population health.
