Chordline Health Launches Comprehensive Disease Management Programs

Chordline Health, a leading provider of fully integrated workflow and analytics software designed by clinicians for managed care, today announces new ACUITYnxt Clinical Care Management Programs. The programs will use patient and population-level data to proactively identify individuals overdue for a health risk assessment (HRA), or those managing or at risk for diabetes, asthma, COPD, heart failure, coronary artery disease (CAD) or complications related to pregnancy.
